Rubber cover came off brake pedal while applying and causing driver's foot to slide onto accelerator pedal, which accelerated vehicle and caused a four car accident. Dealer / manufacturer were notified. The rubber cover for the brake pedal was not glued and the manufacturer should be responsible for this accident.
