Van has shut off while being driven, transmission jumps constantly at 24 mph, radio goes in and out constantly- have no audio 90% of the time, navigation rarely works, car play rarely works, bluetooth drops phone connection consistently, have had rear camera freeze multiple times

Driving on an expressway, the cars in front of me are slowing down, my car's collision mitigation braking system sends a visual alert with the "brake" signal coming on and I feel the brakes engaging, simultaneously I slam on the brakes, and while there was still distance between the car in front of mine and mine, I am not able to brake. The brakes seem confused, and the impact is so strong that my head almost reaches the steering wheel. The seatbelt is locked in place and leaves me sore. Once I am able to get ahold that I was just in a car accident, I am unable to lower the windows, the car is able to call for help on it's own and does so although nothing comes of that in that no help comes. Once I get off the van I notice the plate is knocked and on the floor, the bumper has fallen off and the hood and grill are curled up pictures attached. The car seems to be leaking a fluid and we are unable to start it. I believe that because we (the car and I) simultaneously having the system engaging on it's own and my slamming on the brakes prevented the brakes from working. The impact was with such velocity that it's impossible to have had them working properly. This is an important safety concern if the driver and the system are unable to work together or too confusing for the system. Interestingly, the air bags didn't deploy! additionally, the initial scan by the auto body shop had many alerts come up but the ones I would like to highlight are ABS - ECM failure and integrated driver support system - brake actuator malfunction, attaching that scan too.

When backing up my 2019 Honda Odyssey the vehicle suddenly accelerated at full speed with no input from me. I crashed through a fence behind the vehicle and did significant damage to the vehicle.

Around late August or early September of 2018, we started experiencing some problems with the accelerator. One of our office workers had two accidents while parking due to the malfunction of the accelerator. The accelerator sank down subtly as if she was pressing against sort of a sponge when she steered the wheel at a certain angle. As a result, the car crashed against the wall and we had no choice but to repair the bumper. Originally, we thought the parking accident was due to lack of driving experience of our office worker and paid the full repair cost ourselves. However, a senior executive at my company with 27 years of accident-free driving experiences also claimed multiple times to have experienced the same sponge-like sinking in the accelerator while driving the car.

On December 19, 2018, the day after we had the maintenance and brake fluid repaired, we were driving the car on a freeway. Suddenly, the car shook wildly for a moment and thick white smoke started to issue from the car. To make matters worse, the accelerator did not work, so our car could only reach speed up to 25 miles per hour, much slower than the other cars honking and racing past us on the freeway. We barely made it to the next exit, where we had to park at the nearest gas station to check what was going on. We were under extreme danger and stress from this incident as it happened in the middle of the road and we were extremely close to colliding with other cars. Honestly, no words can dare to express the fear and dread of imminent death we experienced that time. We thought we were going to be crushed to death at that moment. After a few minutes in a nearby parking lot, when we started the car again, the check engine light was gone, and the accelerator started working normally. However, although the problem has seemed to have 'gone away' for now, we do not know when or where it may occur again. It scares us that the car seems so innocently normal again.

(event 1 and 2 happened 10 minutes apart) our new van shifts in the park while driving at highway speeds (62 mph). We took it to freeway Honda the first time and they were unable to render any errors, so the vehicle was returned to us. (event 3) occurred on 12/16/2018 around 11:42 am. We were driving near the 405/5 Y merge and the car suddenly shifted into park. We were able to capture this event on video and here is the direct youtube link to the event: Https://youtu.be/kiztkmoyi-M. car is currently at the Honda dealer where they stated they will replace the transmission. There is also a "crash mitigation error" that appears in our instrument panel from time to time that the dealership is unable to reproduce and cannot fix.
