The contact's wife owns a 2008 Lexus GS 450H. The contact stated while attempting to park and depressing on the brake pedal but the vehicle accelerated independently and hit a lamp post. The contact stated the air bags did not deploy. The contact stated no warning light was illuminated. The contact stated no one was injured nor sought medical attention. A police report was not filed. A dealer was not contacted. The contact stated he experienced the failure for a second time on August 16, 2021. The contact stated while attempting to park and depressing the brake pedal to the floor but the vehicle accelerate independently and crashed into the wall. The contact stated the check stabilizer track and the check system warning lights were illuminated. The contact stated the air bags did not deploy. The contact stated no one was injured nor sought medical attention. A police report was not filed. The contact took the vehicle to local dealer, where it was being serviced at the moment. The vehicle was not diagnosed or repaired. The manufacturer had not been informed of the failure. The failure mileage was approximately 80,000. The VIN was not available.

As I was accelerating from a traffic light, I heard a pop and lost all power to car. I rolled over to side of street, while noticing smoke coming from engine compartment on passenger side. Opened hood and saw a fire by engine wall. I had a couple of bottles of water with me. I pored water on fire and after one minute distinguested the fire.
