As a result, the vehicle crashed into a side embankment, bounced back onto the freeway, turned 360 degrees, and crashed into the embankment again. The vehicle then bounced back a second time onto the freeway before it came to a full stop. Bystanders helped to coast the vehicle over to the side of the road. The air bags failed to deploy. The driver sustained neck and back injuries that did not require medical attention. The front passenger sustained a spinal fracture, eye socket fracture, and back injuries that required medical attention. The rear passenger sustained head injuries that did not require medical attention. A police report was filed. The vehicle was towed to the contact's residence. The vehicle was not taken to a dealer or an independent mechanic for diagnostic testing. The vehicle was destroyed. The manufacturer was not notified of the failure. I am experiencing an anti lock brakes actuator failure. Upon starting the vehicle it gives the warning "stop the vehicle and contact a dealer". I have did this and found that this is an issue that Toyota/Lexus is aware of as they have issued a service bulletin about it and offered an extended warranty. However, the extended warranty is limited by mileage and years. Being a key safety component in vehicle operation, I am requesting this issue be converted into a recall. I have tried to get repair assistance from Lexus customer support and they denied the request based on mileage.

The manufacturer was made aware of the issue and stated that, although the vehicle was repaired once by a Lexus dealer, a Lexus diagnostic must be performed on the vehicle. The contact stated that the vehicle was under warranty for 100,000 miles until February 28, 2016 or until the mileage cap was reached. The failure mileage was approximately 40,000.
