I was at a doctor's office and their parking lot has a large slope. I parked facing up the slope and applied the parking brake. When I exited the vehicle, I felt the vehicle slowly sliding backwards. I pushed rearward on the car to check if the brakes were holding which accelerated the roll. I jumped back in the vehicle and depressed the brake pedal to stop the roll. I replaced the rear brake pads and noticed extremely uneven wear. Additionally, the left-rear caliper parking brake actuator was bound-up and wouldn't function. I exercised the actuator to loosen it up a bit which should be just enough until I can order the part. I experienced this same issue with my parking brake in 2017. In 2017, I took the car to the dealer who replace the rear brake calipers under recall 17V393000. I called the dealer today 8/16/2022 and they stated the calipers cannot be replaced under warranty or recall again. I find it extremely disturbing that the parking brake would experience such an extreme failure twice in the 8 years I've owned this vehicle. In 2017, the parking brake released suddenly while the car was unattended sending the car down my 100ft driveway, glancing my house and crashing through a wooden fence before stopping. Luckily, this time (2022), I know of the prior issue and can be proactive about replacing the faulty part. These calipers need a redesign where the calipers can last more than 5 years before such a safety failure... or that the parking brake not functioning while unattended isn't the first indication of failure.

2-3 months ago I noticed that emergency hand brake was not holding the car again after recall has been completed back in Sep 2017. Now I always leave my car parked on the 1st gear (manual transmission). My mechanic said that calipers have never been replaced and they look original also he showed me that brake pads are not being released and get stuck really hard on the rear axis. I found the invoice and everything that has been replaced is "parking brake lever per recall order". also Mazda declined fixing/replacing brake pads that have been worn out withing 1 year after I replaced them. They said that this can't be covered since the job has not been done by Mazda service and the parts were aftermarket. I had to change pads on the read axis 2 times in almost 1 year and now I have to do it again. Also I hear squeaking noise when I'm driving. I can hear that the pads are not completely released and touching brake disks. Almost a year before the recall my car rolled down the slope and blocked fairly busy road in Washington DC. Luckily it didn't hit any car or person.

A year ago I took my car for a recall that had to do with the handbrake somehow rusting out where it sits in the rear brake calipers. They inspected the calipers and told me they had to order parts. I never received a call. The next time I went back I asked about it and they said it was fix. After the visit they recommended to replace the rear brake calipers. The vehicle is a manual and is to the point where the hand brake does not work at all. It runs the risk of rolling away if the transmission fails or it comes out of gear. It is clearly stated in NHTSA #17V-393 and I have called Mazda corporate who told me it doesn't have to do with the calipers ("your Mazda dealer will check the parking brake actuator shafts inside the left and right rear brake calipers."). Mazda reference number [xxx]. also because of the rust the handbrake is lose so if you use cruise control the handle can accidental pull up enough to ignite the handbrake light and make the car automatically turn off cruise control which is surprising and could cause the driver to crash. When driving, regardless of weather conditions or outside temperature, it feels as if the brakes are dragging. There is a recall on the brakes/parking brake that was issued in June 2017 but Mazda is unable to make the necessary repairs and it is now October 18! this is an ongoing issue so this form is asking "when did this happen?" but this is not just a one-time event. It feels as if the brakes are dragging every day and has felt that way for months.

The contact owned a 2014 Mazda 6. while driving approximately 35 mph in the rain, another vehicle attempted to pass the contact's vehicle. The contact depressed the brake pedal to avoid crashing into the other vehicle, but the brakes failed to respond. As a result, the contact's vehicle crashed into a tree. The air bags deployed. There were no warning indicators illuminated. The contact sustained injuries that required medical attention. A police report was filed. The vehicle was not taken to a dealer. The vehicle was destroyed and towed to a towing lot. The VIN was included in NHTSA campaign number: 17V393000 (parking brake). The manufacturer was not contacted. The approximate failure mileage was 66,000.

After having the dealer tighten the emergency brake cable the E-brake stopped working again so I tightened the cable again and that didn't work. Evidently there thousands of 2014 Mazda6S' with this poor design. There is a flaw in the design of the rear E-brake mechanism. Shame on you Mazda for letting something as simple as this get past your "engineers".

I had my car parked on a hill, it is a manual and I left it running & had the gear in a neutral position with the emergency break on. I was feeding my neighbors pigs when I noticed my car starting to roll backwards so I jumped in the drivers side of the car while it was rolling backwards to stop the car with my foot break. The emergency brake completely failed thank god I told my seven year old son to get of the car while I was helping with the animals! now Mazda is telling me I am out of warranty & I will have to pay to replace both of my rear calipers as they are stuck & replace the rear rotors & brake pads!!! not acceptable! ive taken my car to power Mazda in salem for all oil changes and maintenance and this is the first time they have ever told me that there is a problem with my braking system!!

While driving on the highway with cruise control on, my car atomatically slammed on the brakes and slowed down rapidly. A short time later, the forward obstruction warning indicator came on and said an inspection was needed. Mazda dealership said "never heard of that" despite similar public comments on user forums. Dealer will not cover full cost of the repair becuase care it out of warranty. I believe this is a serious safety issue that should not fall under normal warranty considerations.

I recently replaced my front brakes due to normal wear. I took my car to a local madza dealership, and when the job was completed, my brakes began to make odd noises on the drivers side. I took the car back, and they told me that brakes make noises. I told them, that wasn't true. Prior to the new brake job, my original brakes didn't make odd noises. I then took the vehicle to another madza dealership. Again, I was told nothing was wrong. Meanwhile, when O engage the brakes, there's a strange noise that continues on the drivers side. I've read car forums online, and it said the brakes are the wrong size for these model vehicles. Madza needs to fix the problem before someone gets injured. This happens while using the braking system.

The adaptive cruise control (mrcc), smart city brake system (sbs), and distance recognition (drss) safety features failed for the first time during the mentioned date above (Feb 18 2014, at 21000km. I reported to Mazda service center I khobar, ksa though phone calls, emails, and brought car for several times. I waited for months until the issues were resolved on April 7 2014 at 30000km last June 14, 2014, the same failures occurred again while I was driving on a highway at 75mph. I brought the car to service center on the next day and it took a day to fix the issues. Im afraid the same issues will occur again which is put myself to high risk for road accidents.
