Washer and nut came off the front wheel while consumer was going 15mph. ABS light came on as a warning, and vehicle made a grinding noise. Consumer got home, and found caliper was the only thing holding the wheel. Pin was not put in during manufacturing. Consumer states a flat washer and flat nut (not a lock nut) were used, also there is not a crown nut, nor a cotter pin installed. Dealer stated the bearing had come loose from the hub and failed, probably due to insufficient torque on the flat washer and nut.
