The contact owns a 2013 Chrysler Town and country. While driving 45 mph, the headlights and interior lighting failed to illuminate. In addition, the brake pedal was depressed, but failed to engage. As a result, the vehicle left the roadway and crashed into a tree. The air bags failed to deploy. The contact sustained injuries to the ribs, lungs, and knees that required medical attention. A police report was filed. The vehicle was towed to an independent mechanic and diagnosed, but the cause of the failure was not determined; however, the vehicle was repaired. The dealer and manufacturer were not made aware of the failure. The failure mileage was 38,000.

The dealer stated that the parts were not available. The manufacturer was not made aware of the issue. The contact reported that on one occasion the exterior lighting of the vehicle failed to illuminate. The vehicle was taken to the dealer, but the failure could not be duplicated. The vehicle was not repaired. The failure mileage was approximately 25,000. The VIN was not available.
