Caravan 3.3L V6

Brand new vehicle seat gave way from normal use. How would this end up in an accident? Currently on my third seat. Two Separate issues. (Back loosens and plastic base breaks from repositioning seat for different drivers). Anyone seen fixes that work as I am having issues again outside my warranty this time.
