Failure of driver seat mounting on the window side. The consumer found that the mounting holes, one on each corner of the seat, were retrofitted on the window-side (front & rear) by spot welding a threaded washer about three inches to the right of the corner holes to facilitate mounting of the seat to the rail on the floorboard. This particular design caused metal fatigue surrounding the spot-welded washers, resulting in the seat breaking loose.
