The contact owns a 2010 Mazda Mazda3. The contact stated while driving approximately 40 MPH, the contact made an emergency stop. The contact stated that he struck the steering wheel with his chest. Additionally, the seat belt pretensioner failed to operate as needed. The contact stated no warning light was illuminated. The contact had taken the vehicle to a local dealer, where it was diagnosed and determined that the driver-??s side seat pretensioner Part Number: BCY7-57-L90A-01 needed to be replaced. The vehicle had not been repaired. The manufacturer had not been informed of the failure. The failure mileage was approximately 69,000.
