The contact owns a 2009 Mazda 5. while driving 35 mph, the rear driver side sliding door opened independently. The contact attempted to close the door, but the sliding door failed to latch. The failure recurred on numerous occasions. The vehicle was taken to a dealer where it was diagnosed that both rear door latches needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The contact was aware of NHTSA campaign number: 06V463000 (latches/locks/linkages), but the VIN was not included. The failure mileage was 100,000.
