transmission slips out of gear

So this has gotten slightly worse over the months, starting when driving maybe 1/4 mile to the first light I turn at. Coming to a stop you can feel the car slip into neutral, the car is still in drive... 1, 2, R, all act like neutral. Only way I gain gear is to slightly accelerate to build pressure and it gets into gear... sometimes taking turns is dangerous if I don't feel it slip out of gear... the way I get around it is to actually put it into neutral, rev the sucker up and throw it back into drive when RPMs drop. Then you can feel it shift back into gear... somewhere there's a pressure issue because I've changed the fluid, its at the right amount but continues to act up... few times I've almost been hit trying to turn, accelerate when it acts like it's in neutral and JAMS into gear pulling me into oncoming traffic as it nears... this is very annoying and potentially life threatening. Fluid has been checked continuously and filter has been changed, no fragments in pan or filter... looks beautiful.
