Vehicle was involved in accident on 08/22/2021. The speed was 55-60 mph. The vehicle in front, a 2014 Chevrolet 4wd full size pick up changed lanes (left to right) clipping the left front of the Toyota & slammed on brakes. The Toyota went under the bumper & right rear fender of the truck. The damage to the Toyota included the ECU, ABS & brake components, wiring harness & fuse/relay blocks, & all of the sheet metal, bumper, A/C, etc. Insurance company opted to repair the vehicle (cost is now approaching $18,000). My wife & granddaughter (adult) were the front seat occupants. They both sustained bruises but no serious injuries. If the air bags do not deploy in an accident that destroys the entire front end of a 4 year old vehicle with 25,000 miles (we purchased new), what does it take for them to deploy? Does not give us much confidence in the safety. The vehicle is currently at Greenville Toyota Body Shop in Greenville, SC. Repairs started 09/02/2021. Waiting for interior wiring harness. No warning lights or issues before the crash. The battery was destroyed in the crash.

I was driving in the hov lane on the 170 at 6:30pm. 2/7/2020. There was a sudden white flash and explosion. All of the airbags deployed. I could not see where I was going. All I could see was out my rear window. I tried to steer the car to the side of the freeway but it stopped in the fast lane. All I could see where cars swerving to miss us. I didn't know if my hazards could be seen as I couldn't see my instrument panel. So I pumped the brakes hoping the lights were working and the cars coming could see them. At the same time, I thought the car was on fire because my throat and lungs were burning. My daughter was knocked unconscious. I couldn't move because of the seat belts. When she came to she called 911. The freeway was closed down. The police found no evidence of us being hit or us hitting anything. No report was issued. The airbags had to be cut to get us out. We were trapped in a car that we couldn't tell if it was on fire or not. If it was we wouldn't have gotten out! the car was deemed a total loss. I believe there was a short that may have caused the air bags to go off. Or the airbags just went off on their own. I do not know what the white light or deafening bang was. I can only call it an explosion.

The vehicle was then towed to a body shop and was informed by the insurance that the vehicle was totaled. The vehicle had not been diagnosed as of yet. The manufacturer had not been informed of failure. The failure mileage was 22,060.

On November 4th, 2017, I was involved in an accident which was determined by the police report to be a hit-and-run accident. We were never able to find the person who hit my car while I was driving. My car was severely damaged as a result of spinning across 7 lanes and hitting the sub-dividers. Please see attached pictures of the vehicle below. Unfortunately, none of the 8 airbags in the car deployed. My face was slammed very hard against the steering wheel as a result of the impact. We reported this to the Toyota corporation and they said they needed to do an investigation, which took them more than 2 and a half months, to come to the conclusion that the car performed normally. They sent me some diagrams and other information which was completely incomprehensible. Please see attached documents from the Toyota inspection department. I recently sent a letter to Toyota corporation requiring a fair resolution to my situation. Please see attached copy of that letter. Everybody I show the car to, including body shops and mechanics, felt that I was very lucky to walk away alive from that car. I am extremely scared to drive this car, fearing another accident where the outcome may not be the same. I truly believe this car is a death trap and Toyota corporation is refusing to take responsibility and compensate me for the repairs, car rentals, and other expenses.
