Blazer LX V6 4.3L

This problem is very annoying. When I start the vehicle it shakes a lot. When I start to accelerate the gears will not shift until around 4,000 RPMs. I have changed the fluid several times yet this problem still stays. Mechanic cannot find the problem. Vehicle is also hard to shift from park to reverse.
