After accident found shoulder/lap belt remained latched, but had pulled all the way out, allowing occupant to impact with interior.

Braking to avoid somebody on the road, applied the brakes pretty hard and the child in the passenger seat was in the shoulder/lap, seat belt did not catch the child and the child went into the windsheild with the seat belt.
