The contact owned a 2007 Ford F-150. The contact stated that while traveling at 73 mph, he attempted to depress the brake pedal and the brake pedal broke without warning. As a result of the brake failure, the contact rear ended an 18-wheeler truck. The air bag deployed upon impact however, the seat belt failed to retract. The contact received medical attention at the scene and was later transported to a hospital. The other driver was also treated at the scene but his injuries were unknown. The contact sustained a broken right arm and a broken right femur. A police report was filed at the scene. The vehicle was destroyed and towed to a local junk yard. The dealer was not notified of the failure. The manufacturer was notified of the failure but offered no assistance. The failure mileage was approximately 141,000.

The contact owned a 2007 Ford F-150. While driving approximately 99 mph, the vehicle struck an embankment and rolled over two and a half times. The air bags did not deploy. The driver was wearing a seat belt; however, he was ejected from the vehicle through the rear driver side window. The driver sustained fatal injuries. A police report was filed. The vehicle was destroyed. The manufacturer was not made aware of the failure. The approximate failure mileage was 64,000. The VIN was unavailable.

The contact owns a 2007 Ford F150. The contact was driving approximately 40 mph when the vehicle crashed into the rear of another vehicle. The contact stated that the front driver and passenger side seat belts did not restrain the driver and passenger. In addition, the air bags did not deploy and the contact struck his head on the windshield. As a result, he suffered a concussion and developed post concussion syndrome. The contact also suffered injuries to his back. The contact was able to drive the vehicle from the scene of the crash and the vehicle was taken to a collision center where the body damage was repaired. The vehicle was then taken to the dealer where the technician stated that the seat belt modules were faulty. The dealer did not inform the contact if there was a failure within the air bags. The vehicle was left with the dealer and was undergoing further inspection. The technician also stated that the manufacturer would not assist with the cost of repairs. The vehicle was not repaired. The approximate failure mileage was 42,500.
