The contact owns a 2012 Ford F-150. The contact stated that while driving at various speeds, the transmission would intermittently downshift without warning. The contact then received a recall notice for NHTSA campaign number: 19V075000(power train) and took the vehicle to ray skillman Ford, Inc.(1250 us-31, greenwood, in 46143) where they serviced the recall. One day as the contact was driving at 73 mph, the vehicle suddenly downshifted to first shift and the contact hit his head on the steering wheel. Both rear wheels locked up as the vehicle spun out of control and veered into a ditch. The contact was able to regain control of the vehicle and drove out of the ditch. The contact did not seek medical attention for his injury. Due the failure, the variable timing chain was damaged and was no longer able to be driven. The manufacturer was notified of the failure and they informed him to take the vehicle to a local dealer. The contact then had the vehicle towed back to the dealer where they diagnosed the vehicle with engine failure. The vehicle had yet to be repaired and remained in the possession of the dealer. The failure mileage was approximately 160,000.

The contact owns a 2012 Ford F-150. The contact stated that while driving at various speeds, the vehicle stalled with the powertrain malfunction indicator warning light illuminated on the instrument panel. The contact called dave sinclair Lincoln St. Peters (4760 N service rd, St peters, mo 63376) where he was informed that there were no recalls on the VIN. Upon investigation, the contact discovered NHTSA campaign number: 16V248000 (power train) which he linked to the failure. The manufacturer was notified of the failure but offered no assistance. The vehicle was not repaired. The failure mileage was approximately 188,000.

While driving (at any speed and from any gear) truck frequently, unpredictably exhibits following behavior: - speedometer need drops to zero regardless of actual speed. - truck loses proper shifting functionality. Event has occurred a dozen times or so since 12 December 2020, usually at speeds and gearing above third. Typical result is truck will not downshift below third gear, but on one occasion event occurred at a stop light, and gearing above first would not engage - odometer, trip meter, gas mileage and other digital information displays null out to dashes instead of showing numbers - a wrench symbol appears on right side of the digital display. Symbol sometimes remains for the duration of the event, and sometimes goes away. - sometimes the check engine light appears, sometimes not - airbag light illuminates if I turn the truck off, remove the key, and then re-start, the truck temporarily returns to normal function, with no indication event occurred (check engine light not on, etc). Truck operates normally until it happens again, often within minutes (or even moments) of the reset, but sometimes not again for the duration of a 20 or 25 minute trip. Truck is not safe to operate in this condition and I have now stopped driving it. I have been stuck at a red light on a grade with a load in the bed and traffic behind me, unable to move when the light changes, because first and second gear will not engage, and third gear lacks torque to get moving. Once, after accelerating from a stop, the event occurred and the truck was stuck in first gear, causing unsafe traffic condition (for me and others) while I attempted to pull over safely through traffic. In addition to photos submitted herewith, I have video clips showing occurrence at highway and at surface street speeds. Contact me if you want them.

The contact owns a 2012 Ford F-150 the contact stated while driving approximately 60 to 80 mph, the vehicle downshifted to first gear. The contact stated that the failure occurred on numerous occasions. The check engine warning light illuminated and the instrument panel darkened during the failure. The contact moved onto the road shoulder, shifted into neutral, turned off the vehicle and opened the door to reset the computer. The contact then unlocked the door, restarted the vehicle and the vehicle resumed normal operation. The contact performed a self diagnostic test on the vehicle and retrieved dtc codes: P0722, P1500, T0720 which confirmed a loss of communication between the speed sensor within the transmission. The vehicle was taken to don rich Ford company Inc (221 E montgomery St, villa rica, ga 30180, (770) 459-5132) to be diagnosed. The dealer declined to re-flash the computer as requested by the contact. The manufacturer was contacted. The vehicle was not repaired. The approximate failure mileage was 139,000.

While driving, either going 25 mph in a housing development or 75 mph on the interstate, the truck will sometimes shift gears, throw a wrench light, and the speedometer will read 0. there has been a recall for an issue similar to this for trucks between 2011 and 2013. Trucks not covered under this recall were given a feild service action for extended warranty. They know this is an issue to more vehicles than what they recalled. Owners were not notified of this field service action. Not only that, but this is very dangerous while going 75 mph on the interstate. This causes the truck to down shift, slow down, and you must pull over for safety on the interstate. A code will pop up as P0720 or P0721. Ford should be doing more since they know it is an issue, or they would not have extended the warranty. This is happening to many F-150 vehicles built in those date ranges.

When driving at a normal highway speed of anywhere between 50 to 75 mph the transmission will down shift to 1st gear. Does not suddenly slow down because the gear returns to 5 or 6th gear immediately. I have had a Ford dealership address the issue by reprogramming the control module on the transmission but I am still experiencing this problem. I believe it to be a high safety risk. The occurrence happens intermittently. So there is no specific date available.

The contact owns a 2012 Ford F-150. The contact stated while driving 30 mph approaching an intersection, the vehicle jerked and loss motive power without warning. The contact restarted the vehicle however, the vehicle failed to resume normal operation. The failure recurred intermittently. The dealer bob thomas Ford Inc (2215 dixwell ave, hamden, ct 06514) was contacted and informed of the failure. The contact was informed that there were no recalls on the vehicle. The manufacturer was made aware of the failure and informed the contact that the warranty had expired. The contact referenced NHTSA campaign number: 19V075000 (power train) as a possible solution however, the VIN was not included. The failure mileage was approximately 190,000.

Multiple times while in motion 45mph + (spontaneously), orange wrench comes up on my dash, speedometer drops to zero, engine power is cut to almost nothing, and a few times at higher speeds 60mph +, the wheels lock due to transmission downshifting out of nowhere. This has happened on the loop, city streets, highways, it's random. I can only limp my way to a safe spot to pull over, turn engine off then back on and everything generally starts working again. Last night on a city road doing about 55mph it locked up again but refused to drop out of 5th gear though I was at a dead stop. Even turning the key of and on would not re set it. It finally got to 1st after messing with it and I got it home, hooked it to my obd2 reader and it is throwing a P0720 output speed shaft sensor malfunction code and my check engine light will not go off. I have attached a picture of the code it is now showing, and date of malfunction is most recent.

My truck was 10,000 miles out of its extended warranty when on cold start up the engine would rattle on startup. I took it to the Ford dealer in my area and was told the timing chain and cam phasers would need to be replaced. The dealer had my truck for 2 months. With parts on back order. With the transmission service I had done the total bill was $3,085.51. The truck was regularly serviced every 5,000 miles by a Ford dealer. The cause of the problem was not lack of maintenance it was an engineering flaw from the manufacturer. Due to this I feel like I shouldn't be financially responsible.

The wrench symbol comes on and the digital instrument screen goes blank and the transmission either gets stuck in 5th gear or will start off in 3rd and only shift through 5th gear. You can turn the truck off and restart it and most of the time it will reset and start off in first and the screen will return to normal. Unusually when this occurs when starting to drive again it will not shift out of first gear and at this time you have to disconnect the battery and reconnect after 15 seconds to regain mobility. I have replaced the PCM to no avail. The new PCM is registering a code now for the output shaft speed sensor which has had a recall in other 2011-12 F150 models. Through research and conversations on Ford truck forum's I have learned a lot of other people are having the same issue.
