The contact owns a 2001 Ford F-250. While driving at an unknown speed, the vehicle crashed into a pedestrian bridge. The driver was fatally injured. The vehicle was towed to a dealer where it was diagnosed that the brake pedal was bent and overlapped the accelerator pedal. This issue may have caused the crash. A police report was filed. The vehicle was towed. The manufacturer was made aware of the issue. The failure mileage was approximately 109,000. Updated 04/04/17

The brake pedal and the gas pedal are too close, and gas pedal is 1/2 inch lower. When stepping on the brake pedal consumer can hit the gas pedal at the same time. Dealer states no modification because of design specifications. This problem would become worse in the winter when the consumer wears boots.

Consumer was pulling out of an intersection onto a four lane highway. The consumer floor the accelerator pedal in order to get up to speed with oncoming traffic when the vehicle suddenly idled down. The vehicle was taken to the dealer who determined that the throttle stopping device on the pedal assembly bent when the pedal was floor and caused the truck to idle down instead of up. The steering wheel did not respond when the consumer slammed on the brake.
