Cpsc: Baby trend hybrid LX 3-in-1: consumer stated she and her son, were involved in a head-on collision. After the accident, the consumer noticed the chest clip had un-latched and her son was dangling out of the car seat and was barely in the shoulder straps, despite being correctly placed in the car seat. The consumer's son passed away from his injuries. Model # FB48417. See also ODI 10855262

I was traveling from las vegas, nv to twin falls, id with my son [xxx] on Dec 14 2015 to visit family for christmas. My son [xxx] was a week from turning 4, weighed 37 lbs and was 38 inches tall. I had him in the baby trend hybrid 3 in 1 LX (kiwi color) forward facing in the 5 point harness. At around 9pm mountain standard time on Dec 14 2015 my son [xxx] was able to pull the slack on his harness loose after unclipping his chest clip and freed his upper arms. The moment I noticed he had done this our small car was hit head on by a pickup truck that was driving at a high rate of speed. Since [xxx] lower body was the only thing restrained his upper body was thrown forward at a high rate of speed which caused a hyper extension and severance of his spinal cord at the C3- C4 vertebrae. The contact owned a 2014 baby trend hybrid 3-in-1 booster seat. While driving 25 mph, another vehicle crashed into the rear of the contact's vehicle at a high rate of speed. The driver sustained whiplash and the child passenger was fatally wounded due to the impact of the crash. The seat belt did not secure the child during the crash. A police report was filed. The vehicle was towed to a yard and the booster seat was discarded. The contact could not diagnose what caused the failure. The manufacturer and the insurance company did not provide further disclosure on the investigation of the crash.
