I have steeltex at LT245/75 R16 tires on my 95 Suburban. Tires purchased in 2001. Passenger front blew out 2/20/04 while traveling on hwy 10 traveling about 65 mph. I have heard about a recall. What forms do I need to complete to get a refund/ exchange of all tires?

While driving on the freeway at about 65  70 mph, the tread separated from the sidewalls of the tire. In a matter of about 15 seconds we were riding on the rim. Because it was a rear tire, I was able to maintain control of the vehicle and pull to the side. Had it been a front tire, I doubt that would have been the case. We were headed out on a trip and I had checked the tire only 15 minutes before. Everything looked fine. I have photos of the tire and have saved it and the others from the vehicle.
