I had intermittent warning lights about the electronic parking brake and steering assist for lane departure. One day I left my house and warning lights came on about brake failure. There were no power brakes and the little braking available was very not good at all. I had the car towed to a dealer and it was diagnosed with an internal failure of brake control unit code pfe2b oo. I had the unit replaced.

The contact owns a 2020 Honda CR-V Hybrid. The contact stated that while his wife was driving up the driveway, at a slow speed, the brake pedal was depressed. The vehicle then accelerated independently. The auto stop feature failed. The contact's wife veered to avoid hitting a parked vehicle. However, the vehicle hit the side of the garage and hit a small tree, where it came to a stop. The contact advised that a branch went under the vehicle. No warning lights were illuminated. The driver-side and passenger-side air bags did not deploy. The contact's wife suffered injuries to the front part of her head. The contact was occupying the front passenger's side seat at the time of the incident. Medical attention was not received. The police were not notified. The vehicle was towed to a collision center but was not diagnosed or repaired. The insurance company was contacted. The vehicle was towed to the residence. The manufacturer was made aware of the failure. The manufacturer advised the contact that a case manager would be assigned. However, the contact was awaiting to hear from the case manager. The failure mileage was 42,332.

The contact owns a 2020 Honda CR-V Hybrid. The contact stated that while driving at various speeds, the front driver-??s seat and the front passenger-??s seat felt hard and uneven. The contact checked the bottom of the seat and became aware there was a space between the seat rails and the carpet. The vehicle was taken to the dealer, where the carpet was shrunk to size. The contact stated that on a separate occasion, while driving at various speeds, when the brake pedal was depressed, the contact-??s foot slipped off the pedal, compromising the brake prevention system. The contact stated the brake pedal is higher than the accelerator pedal. The vehicle was taken to the dealer; however, no cause for the failure was found. The vehicle was not repaired. The manufacturer was notified of the failure but denied assistance in covering the cost of the repair. The failure mileage was approximately 100.

Brakes failed due to a known issue with the Brake Simulator. The same vehicle has been recalled in Japan for this reason. Brakes became completely non-responsive while driving. Again, this is a known issue. The brakes simply due not work and in Japan they have identified a manufacturing process defect. [XXX] Vehicle was inspected by Team Honda in Baton Rouge. Brakes stopped working suddenly. All of the brake system started flashing warnings. electric parking brake light is on when parking brake is not engaged. Warning lights all flashed: Electric Parking Brake Problem, Brake System Problem, Brake Hold System Problem, Adaptive Cruise Control Problem, Collision Mitigation System Problem, and Road Departure Mitigation System Problem. This car has 50k miles and just out of warranty. It was a $6k repair!!!!! Honda said faulty parts.

Yesterday (XXX) while starting my car the warning lights came on notifying me of a potential issue with multiple safety systems including the brake system.? My parking brake would not show that it was released and when put in gear the brakes were very mushy and the vehicle did not feel that it could be driven safely.? I requested Triple AAA to tow the car to the Honda dealership in Lumberton North Carolina this morning (XXX) as I did not feel that it was safe to drive.? A diagnostic test was performed that indicated that the Electric Servo Brake System / Electric Brake Booster System Malfunction was found and that the vehicle needs an Electric Servo Pedal Simulator Unit replaced. This component functions cooperatively with the Regenerative Brakes together with a structure in which pedal force is not influenced by braking pressure control. I have researched this issue and found that this particular problem and the associated part has been recognized as an ongoing problem with some Honda Hybrid models and that it has recently been recalled in Japan, and many Honda owners in the UK have also been experiencing the same malfunction to this part.? This appears to be a systemic issue with a part involving select Honda Hybrid models that appears to occur as early as after 20,000 miles and oftentimes shortly after the vehicle involved is out of warranty.??American Honda has not officially recognized this problem as a safety issue. The part that failed is called the "ELECTRONIC SERVO PEDAL SIMULATOR UNIT", aka "SET, BOS ASSM." aka "BRAKE PEDAL SIMULATOR (SIMULATOR SET, PEDAL FEEL)" aka "BRAKE PEDAL OPERATING SIMULATOR". The Honda (OEM) part number is 01469-TPG-A95. The part failure was diagnosed at Buckeye Honda in Lancaster, Ohio, in coordination with the Tech department at Honda corp., and was replaced at Buckeye Honda in Lancaster, Ohio. The Buckeye Honda (service department) said that they had not seen this problem before, so they contacted the Tech Dept. at Honda to seek assistance in the diagnosis. The safety of the vehicle occupants was put at risk by the almost complete loss of brake functions and the interacting functions. At first the Brake warning and AVS warning lights would illuminate on the driver's display, and then they would go off and everything would function normally. But the driver' display warnings got more frequent and the brakes would not function normally. Finally, all of the driver's display warnings would sequence through, one by one, and the brakes were almost entirely gone. The Honda dealer did not give me the faulty part because he said it would have to be sent to Honda. I do not know if Honda America will inspect the part to determine the cause of the failure. I did take a cellphone picture of the part. The problem began approx. [XXX]. I picked up the repaired vehicle at Buckeye Honda on Monday [XXX]. The part was out of warranty by about one month: The "In-Service date" was [XXX]. The mileage on the vehicle was 12,920 miles at the time of repair and I am the original owner. Honda America did give me a "Goodwill" authorization number, in coordination with Buckeye Honda in Lancaster, Ohio, which lowered my out of pocket cost to $l457.73. Started the car and drove out of garage and had break failure. My wife was driving and she said that when pushing hard on breaks there was very little break ability (at speed there would be none) At this time a bunch of lights went on warning about break failure. Fortunately this happened at the initiating of driving. (A similar account is in the uploaded document). According to Honda the break operating simulator failed (the equivalent of a master cylinder). Honda replace the breaks with the same part and as I was told there was not updates or fixes to the faulty parts. The car had all it services and are really well looked after it has only 25000 miles on the clock. No modern day car should have break failure after 25000 miles. New vehicle with approx. 3,000 miles became completely disabled on highway and exposed occupants to extreme danger. Dealership reported that a "faulty brake sensor" was the cause of the vehicle becoming inoperable. Vehicle was being driven at 60 mph and engine died on an underpass turn barely visible to traffic behind and was almost hit by a semi cruising at 60+ mph. Failure of all systems resulted and could have killed the Honda CR-V occupants.
