The shoulder harness separated from the yolk on the rear of the child safety seat. The owner un-installed the safety seat and re-attached the shoulder harness to the yolk. Owner later re-installed the seat into the vehicle attaching the seat belt of the vehicle and attaching the seat belts of the vehicle to the tether. Consumer pulled on the adjusting strap, but it was tight. Pulled on the harness strap again and it came loose in consumer hand. It had somehow bunched together and came off of the yoke behind the seat.
