At first, some dash relay that normally lights the D4 transmission indicator lamp briefly at ignition-on was not doing so, resulting in no engine start. All electrical systems remain working, including starter motor. Best guess is that something is causing a complete electrical shutoff of only the spark or only the fuel (pump). Only seemed to occur every 10 to 15 starts or so, and when engine was already warm. After 3-5 minutes, relay kicks on by itself, D4 lamp lights briefly, and engine can then be started perfectly normally, first try.
Eventually, the same problem (evidently) started to occur WHILE DRIVING, completely shutting the engine, and preventing restart as above.
Honda service could not produce the problem in-shop, advised to replace ignition switch which we did. No fix to the problem; in fact the engine cut while driving away from the service shop.
