This issue has been occurring intermittently for over three years. My passenger headlight goes off randomly while driving, and the headlight switch must be turned off and then on again manually for the light to come back on. As of three months ago, now BOTH my headlights will randomly turn off while driving, and, again, switch must be turned off, then on again, manually for them to come back on. VERY disconcerting when driving on a dark road at night and both lights just disappear, and DEFINITELY a safety hazard.
When first taken to a dealer, said i would need a new headlight for around $400--totally unacceptable, and I declined. Now that both headlights are doing this at once intermittently, I seriously doubt that replacing the headlight is the fix. This has been reported in numerous other Lexus ES 350's, and I believe there should be a formal recall for this issue.

The headlight on the driver's side works when I first start the car. After driving for a while, it goes off. So the bulb seems to be ok. Not able to figure out what the problem is. Worried about getting cited. Is there something in the controls that is heat sensitive that is tripping a switch as the car warms up?
