At approximately 6:45pm on Tuesday, December 27, I was driving in Nassau County, NY, when my sunroof exploded. The sound was terrifying and I was very concerned that I was shot at or hit by an unknown object. At first I thought the object hit my car or my back windshield. I looked and did not see any holes anywhere but I could hear air coming into my car. I moved my sunroof shield and saw a gaping hole and shattered glass.

DAMPER SERVO SUB-ASSEMBLY, AIR CONDITIONER RADIATOR, NO. 2 Part Number: 8710608120 Which also called the bend door actuator failed on my 2017 RX450h at 80,000 mile.The part or door opens to mix hot air form the heating core and cold air to supply heat to the interior of the vehicle. When the bend door actuator part failed you have no heat and to replace it you have to he the dash removed (poor engineer design) and face bills for service over 6,000 dollars. This part failed appears to be prevalent with this manufacturer. Other manufacturers such as Mercedes benz, Porsche, and Honda etc. do not appear to suffer from this part failure problem at the same mileage. This appears to be a lemon part form this Manufacturer. The on board mapping system is no longer accurate for speed limits and as such ( assuming speed limits were lowered for safety reasons) creates an unsafe driving experience. Other manufacturers update there mapping technology at no cost ( eg Audi) Lexus require $500. It needs to be free.

When checking the emergency SOS function in my car I found that it did not work. I called Lexus support and was told that the following safety systems would no longer be operative: Lexus Enform Safety Connect including Automatic Collision Notification; Enhanced Roadside Assistance; Emergency Assistance Button and Stolen Vehicle Locator; Lexus Enform Remote including Remote Engine Start/Stop, Door Lock/Unlock, Vehicle Finder, Guest Driver Monitor, Vehicle Status and Alerts, Key Fob Remote Start, and Integration with Alexa® and The Google Assistant; Lexus Enform Destination Assist including 24-hour live response center agent; Lexus Enform Service Connect including Vehicle Health Report, Vehicle Alerts, and Maintenance Alerts; These include safety systems that I rely on, especially when driving on interstate highways, and were included when I purchased my car. I had no warning that the services would cease to function in October of this year. Lexus states that these systems will no longer function because telecommunications providers will no longer support 3G service. This is certainly a safety issue for me and for other drivers and I believe that Lexus should be required to modify the hardware/software in these vehicles to function in a 4G/5G environment. Lexus representatives state that Lexus has no plans to correct these safety issues. I am requesting that NHTSA review this matter as a highway safety issue.

Sunroof Glass Failure Report: I was traveling on the interstate at the speed limit and there was a loud bang, which after I pulled over, discovered that the sunroof had "exploded". There was no sign of anything hitting it and the weather was typical for the Texas Gulf Coast in a July morning. Luckily, the underneath cover for the sunroof was closed, because several pounds of glass fragments were released in the sunroof failure and would have been all over the passenger compartment of the Lexus. This is a major safety issue, especially if a child is fastened in a car seat with this problem directly overhead. This issue has been reported by numerous other Lexus owners and seems to occur often enough that it should be a concern for the manufacturer, but I am not aware that they have taken any action on this. A photo of the failed sunroof is included.

I hear a loud clicking sound that is coming from the vacuum pump that sends vacuum to the brake booster, I am concerned that it could eventually cause braking issues.in the future if not corrected, i have read multiple forums and it seems to be a common problem with this particular vehicle and ones similar to it, I would like to have this issue resolved before a traggedy occurs.

I when to a car wash facility and the ics sensor when crazy!!! (flashing the warning light). Now my car giving me a ics malfunction. The automatic braking don't work and the parking assistance neither. I believe this is a manufacturing problem due same car model and year experience the same but it has different VIN so there is not a recall for my VIN.

Blind side monitor on right hand side does not alert when passing trucks and cars at highway speeds. At lower speeds it works properly. When passing vehicles on the left the left side warning light comes on properly.

Pulled into hotel parking lot at 11:05am (perfect weather - parking lot empty) and, after turning into the parking spot at a slow speed, the car suddenly accelerated and hit the side of the hotel. The building wasn't damaged, but the car had serious frontal damage. The hotel manager didn't call the police so there isn't a police report. The air bags did not go off. I feel there were several possible Lexus problems with this incident: The air bags did not go off even though there was over $17,000 in repairs (even the engine had to be removed),; the automatic braking system didn't engage; and the sudden acceleration of the car from a slow entry into the parking space. Currently the car is at a Lexus dealer waiting for the Lexus hired inspector to examine it. We have pictures and a complete list of repairs if requested. W E do not have a report yet from the Lexus inspection because it hasn't been inspected yet......

This is a supplemental entry to modify complaint 11115465. It should be noted that the heads up display reflected sunlight directly into the drivers eyes causing serious distraction and a serious safety concern. Lexus corporation was notified of the situation through the dealership but did not offer any fix to the situation and the dealer reported that they were therefore not allowed to fix the reflection and remove the hazard. The complaint to Lexus is documented and more information can be provided upon request.

The heads up display unit causes a distracting reflection into the drivers eyes that interferes with vision while driving. This occurs when the car is stationary, in motion, on a city street and highway, and/or turning. This was not identified until after the new car was purchased.

I leased the vehicle at the end of December 2016. Sometime in September, my wife started to notice an odd smell in the cabin. On sat 10/7, my wife noticed the front carpet was completely saturated. I gave it a few days to dry out but when it didn't get better I brought it in for service I explained the issue to the service manager and without even looking at the vehicle he said he had a feeling he know what it was. He explained that spiders get into the ac drain plug and clog it causing water to backup into the cabin. The padding and carpeting would need to be replaced and mold remediation would be needed (the mold is what was causing the odor). He shared they just did three of these repairs in the past several weeks and told me the repair would cost around $5,400. He continued to explain that this was not covered under the warranty and that I would have to put a claim through on my insurance. After two days he came back and confirmed a "spider infestation." I questioned him since I found this whole scenario odd. He told me he didn't have an exact number but that the dealership had previously performed between 15 and 20 of these repairs and that to prevent a recurrence they would fit the drain plug with a special insect repellant tip. I considered this a reoccurring issue that Lexus was aware of and that they had a fix for so I did not want to put an insurance claim in and risk a rate increase. I then escalated the issue with Lexus corporate and they sided with the dealer saying it's not covered under warranty but that they would cover my comprehensive deductible (which they did). After back and forth with my insurance company they finally approved the claim (they initially didn't want to cover the dealerships labor rates). After having the care for three weeks, they finally begin repairs.

Pre-collision system failed. Vehicle rear ended another vehicle. No warnings. No auto braking. Full unrestrained impact. Collision occurred on freeway off ramp. I have photos of crash site and a copy of the data from the "black box" which Lexus downloaded. The allstate claim number #0453405540. The chp office at 619-401-2000 has incident ncic#9680, by officer id 18801. The Lexus was totaled and allstate paid out. I purchased the Lexus primarily for its safety features. I'm concerned that further faults could cause great harm or death.
