The contact called on behalf of his daughter who owns a 2015 Mazda CX5. The contact stated that while his daughter was loading groceries in the trunk, the liftgate dropped unexpectedly and the corner of the liftgate hit her in the head, causing a laceration. The contact's daughter was taken to urgent care where she was informed that she had sustained a laceration and three staples were put on the laceration. The dealer was notified of the failure and an appointment was scheduled. The vehicle was not diagnosed or repaired. The contact stated that the vehicle was previously repaired under NHTSA Campaign Number: 16V644000 (Structure); however, the recall remedy failed to resolve the safety issue. The manufacturer was notified of the failure and informed the contact that the recall repair was one-time free of charge and they were not obligated to repair additional failures. The failure mileage was approximately 103,000.

Was turning right onto side city street to park, was going maybe 2 miles an hour because I was going to park. The car engine self accelerated and I could not control it, hit a chain link fence and sheared off a natural gas meter to a home. The break pedal did not stop the car, I was in drive put into park and shut the engine off

My lift gate failed and slammed shut. Now, the lift gate is nearly impossible for me to open and I have to hold the lift gate open myself. Otherwise it slams shut. Luckily when this happened I was not directly underneath the lift gate, I had free hands and reacted quickly enough to catch it. I did have the lift gate supports replaced already on July 1, 2017 as a result of the recall 16V644000, but it appears the replacement parts also had the issue. This is dangerous, and a little over one year is not an appropriate lifetime for these supports given the potential risk. The vehicle was parked.

Driving down the highway and heard a loud boom sound. Next thing I know, pieces of the sunroof start flying off the car. Nothing was around the car and nothing hit the car. The only damage was to the sunroof glass and the glass was bubbled out, not in like you would expect if something hit it.
