The contact owns a 2015 Mazda Cx-5. while driving 25 mph, the headlights failed to illuminate at night without warning. The contact stated that the vehicle was turned off and restarted several times, but the headlights failed to illuminate. The vehicle was taken to whitten brothers Mazda (10745 midlothian turnpike, richmond, va 23235, (804) 378-8200) where it was diagnosed that the headlight assembly needed to be replaced. The vehicle was repaired, but the failure recurred. The manufacturer was not notified of the failure. The failure mileage was 34,000.
