I am submitted this report for the 12/6/2024 event. The form I submitted previously was for the safety incident that occured last night (1/9/2025). On four different occasions, the most recent being last night (1/9/2025), the vehicle has suddenly lost drive power while driving. The ability to accelerate is suddenly gone. When you press the gas pedal the engine revs, as if it is in neutral, but it does not accelerate. When this happens, the vehicle is still in drive as indicated by light on the shifter showing drive and the fact the vehicle, just moments before, is driving. During all four instances, I had to bring the vehicle to a complete, put the car in park and then back into drive. On three of these occasions there was no pull-out lane and I had to bring the vehicle to a complete stop in a moving lane of traffic. The incident last night occurred mere seconds after making a left-hand turn with traffic coming up behind me at a higher speed then I was travelling. My safety, the safety all passengers in my car and the safety of other drivers are all at risk when this occurs. When this happens there are no warning lamps or messages that appear. After the third time it happened (on 12/6/2024), we took the vehicle to the Mazda dealership we leased it from and it was inspected and we were told they found nothing wrong with it. They noted they did some updates. They kept the vehicle from Monday 12/9 - Thursday 12/12. Less than a month after getting the vehicle back from the dealership this issue occurred a 4th time (on 1/9/2025 at about 8pm). We will be calling the dealership this morning and setting up an appointment for them to again inspect the vehicle for this safety concern. The vehicle is available for inspection.

I am concerned about a serious safety issue with my CX-90. Both of the rear doors have consistently malfunctioned, failing to latch properly when closed. The door handles get stuck and do not fully return to their original position, preventing the doors from locking securely. As a result, the doors have swung open while the vehicle was in motion, putting passengers, especially children in the back, at significant risk. This is a critical safety concern that needs immediate attention. This issue goes back mid November 2024.

CX-90 AC system is faulted, while driving it keeps the temperature at comfortable level. However, it starts to blow warm and humid air at idling or stopping at traffic or traffic lights (with i-stop disabled). When gear is shifted into Park or Neutral, AC starts working normal again. This causes uncomfortable and unpleasant driving experience in hot summer months. The complaint has been filed with Mazda Corporate, but no resolution to this issue. Hopefully, this will be investigated by Mazda Engineers.

On [XXX], I was heading home from Brooklyn. It was a rainy day. The vehicle was functioning fine when all of a sudden the sign on the dashboard said "Iactive sense deactivated" which is something that always happens when it's raining. It had happened the Thursday prior to the accident when heading to the city and many other times. I had just crossed the George Washington Bridge, on [XXX], going the speed limit or less, when the the iactive sense sign came up. About 1000 feet after I saw the sign and repeating it to my daughter the vehicle tried to get into the right lane, I was in the left lane of three lanes. I knew it was the iactive sense because i had felt it before many times. I braked and controlled the vehicle back into the left lane. said, "I can see the lane" The vehicle then sped on, moved right, then sped all the way right into the guardrail, all the while I tried to brake and turn left to avoid hitting the guardrail but the wheel was locked and the brake did not work. My right arm got hit by the air bag deploying. After hitting the guardrail we span all the way back into the left lane with the vehicle facing the opposite direction facing on coming traffic. I then hit the hazard lights a few times out of panic, then I had my daughter crawl to me (she was sitting behind me) in the front so we can get out because I thought there was smoke. Turns out it was the airbags' powder, according to the fire department. Another lady who was from the NYPD stopped to see what happened and I explained to her what happened and then the NJ police showed up and she left. When the police got there, I told him what happened. He repeated, "you lost control" at which I said "no the vehicle took control" From my observation the vehicle was like was angry and confused about where to stay and hitting something was the only way it would stop. I called Mazda, they said let the police and insurance take care of it. Geico said they don't read vehicle INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S

Mazda's Driver Personalization System is highly defective and properly recognizes a driver, at best, 40% of the time. Since multiple functions are tied to this system such as seat position, steering wheel position, infotainment presets, and a variety of other vehicle preferences, this not only causes an inconvenience issue but also a safety issue. The safety problem is that one has to stop the car, put it into Park, press through multiple menus and keystrokes just to change the setting to the correct driver.

Mazda Connect (Infotainment System) will regularly freeze and reboot. Issue is more pronounced when more than one iPhone is connected via USB-C. If each phone has a different version of iOS, Mazda Connect will freeze, crash, and reboot. If one person is connected and then physically disconnects, then another iPhone with a different version of iOS connects, Mazda Connect will freeze, crash and reboot. To make matters worse, these incidents are not recorded in any logs.

I was driving in EV mode and was behind another vehicle waiting to turn out of a plaza. The exit is on a slope which is important to mention. The car in front of me began to turn and as I was releasing the brake to move up the hill, the car engine kicked on, the RPM-??s went up extremely high as if the accelerator was being pressed all the way to the floor, the car jolted forward on its own and hit the car in front of me. I kept trying to brake, but the car continued to accelerate on its own into the car in front of me a second time. I understand that when on a slope driving in EV mode, the cars engine will kick in to allow the car enough momentum to make it up the hill. However, in this case, the engine kicked in, but the momentum was extreme to the point that when I released the brake to roll forward the car shot forward so fast there was no avoiding a collision. This is extremely faulty and unsafe! My child and I suffered injuries as a result of striking the other car twice. We-??re lucky we were not hit by multiple other cars as we were propelled into the intersection. I-??m terrified to drive this car with my children. I contacted the dealership that instructed me to have the collision repairs completed first and then they would check out the car out.

Driving up snowy pass on I-90 approaching the top of Snoqualmie Pass. Vehicle stalled and warning lights flashed critical warning and loss of power. The vehicle stopped in the middle of the highway and would not start or go into drive. After a few minutes, because of speeding and adverse conditions, another driver collided with my vehicle from behind. Another following vehicle lost control and ran off the road. Nobody was injured. Mazda has not provided any information to what happened with the vehicle. We added about 12k miles in the 5 months of driving it, so we had lots of testing with it, and never ran into this serious of an issue with this or any other vehicle.

1st instance 12/06/2023- The battery was drained and it won't start so they changed the battery. Dashboard light says engine malfunction have the vehicle inspected. 2nd instance 02/10/2024- same dashboard light Engine malfunction and Hybrid malfunction. This time they said that DTC was P0E10. They also noticed low battery charge warning. Battery was charged with DCA8000 charger and they said that notif went away. 3rd instance-02/23/2024 same dashboard light. This time they had DTC's of 5. P0E10, P1739:12, P1B05:16, P1739:00 and P1B05:00. to think this is a brand new card purchased last october 2023 with less than 3000 miles.

Started hearing a rattle in the mid console of the car that gradually got worse. When i checked under the car, there was a bolt missing and one hanging that was suppose to be holding the propeller shaft. When i took it to the dealer, they tried to lie and say that they were only loose which is what you will see on the paperwork but when i confronted them that i saw one was missing and the other bent, they said yes, they don-??t know what happened but they fixed it.

The -??360 View Camera-?? image periodically goes blank, providing no image, just a blank screen with the colored lines showing vehicle and wheel direction. So when the car is put in reverse or forward, no image of the surroundings are visible. This will not resolve until the vehicle is turned off for an extended period. (An immediate restarting of the car will not resolve.) This car has had the other camera recall addressed and another camera TSB addressed. The issue continues to occur at random, unpredictable times. It has perhaps occurred 10-11 times.

The lane assist safety feature stops working. The car will allow drifting across well-marked lanes, despite being a clean car and a clear weather day. The car will not attempt to keep the car in the lane. The events are unpredictable. I have checked the settings when it has happened and they all appear to be the same - the lane assist and related boxes remain checked, etc. After turning off the vehicle for an hour or two, the system seems to reset and work again. This has happened twice that I know if, but could have happened at other times that I did not notice.

The car has an "Engine Malfunction" emergency light that goes on. I brought the car into the dealer right away - the car had $1k miles and is brand new, so it's quite scary to get this message. They cleared the error and told me not to worry about it, but were unable to provide a solution. It recurred immediately (within 1-2 days). I reached out to the service department and they told me to ignore it (that there is no fix in progress). My car is unable to change the Mi Drive mode (so it can only stay in Hybrid mode). This is unsafe and goes against the intended use of the car. For example, on a very cold day like today, I wish to warm the car in sport mode (aka not hybrid or electric) and I was unable to change the mode.

Have to turn it down to 62 degrees to maintain an acceptable temperature. This is in the Midwest with outside temperature below 30 degrees and climate control set to auto.

On 12/5/2023, upon turn after intersection, loss of power to the engine. Pushing gas provided no response from the engine. Engine power cut to about 10 miles an hour. Felt as the car was driving on battery - as no engine was heard. Was able to pull over to the side. Turned off and on engine 2-3 times but the power wasn't restored. Received warning on dashboard that there is malfunction with engine. Also received errors on Mazda application I had on phone. Engine light was on the dashboard. Photos attached. While obtaining assistance for tow service I was accidentally disconnected. I tried turning the car on again, and engine turned on normally. Total time between last unsuccessful turning on/off engine was about 10-15 minutes. First time experience such issue with loss of power and engine cutting off/ not working. Informed the car dealership and car repair/ check was scheduled to be brought to dealership for 12/8/2023. Estimated repairs taking 4 to 5 days before I can get the car back. Distance to dealership about 150 miles. Confidence in this car severely shaken - considering the price and the fact it is brand new.

After turning vehicle off, two incorrect warnings appeared on the dash monitor. 1) -??Rear Seat Alert. Check Rear Seats-??.-?? 2) Warning image and sound regarding the rear bumper. However, there were no objects in the back seats (except for a light sunhat that had been there for weeks), and there was nothing near the left rear bumper. In fact no objects for probably 10 feet from the bumper. I noticed this once, 10/24/2023 but it may have occurred before and I didn-??t notice (I frequently get out of the car quickly when at my destination). Safety concern since the safety features are not giving correct warning signals.

Gear shift can easily slide from Drive to Neutral (and from Neutral to Reverse) if a book, purse, or other object is on the center console and slides forward and touches the gearshift when vehicle comes to a stop. Photos attached show me shifting from D to N with my pinky finger. This is a safety problem because if the center console is flat, making it easy for someone to place an object on it. When the car needs to stop somewhat quickly, the object on the console slides forward and pushes the gearshift into Neutral. This happened to me while I was in traffic, resulting in confusion as to why I could not drive the car forward after my stop. I then realized the car had been shifted into neutral. I shifted the car back into Drive and removed the object from the console. Safety issue for anyone beginning to drive this car wi the out this knowledge. Could also occur if a passenger (unknown to the driver) places an object on the console.

Safety issue, random, with 360 View Monitor cameras malfunctioning. This camera issue has happened randomly at least 7 times since I purchased the car in May 2023. The most recent occurrences were 10/26, 09/27, 09/23, 08/03/2023. What happens: when starting the car, 360 View Monitor should show images of surroundings. Instead, no image from the cameras shows; it-??s black on the screen. Colored lines show the direction the car is backing up or moving forward, but the rest of the screen is black. When it occurs, it will remain in this faulty condition for the entire drive. The button on the console (left of gearshift) that turns on the image cameras when driving will show the same image. It will remain in this disabled mode until the car is turned off for a long time. I-??ve tried turning it off and letting it sit until the computer sounds like it has shut off (2-3 min, perhaps), and restarting the vehicle -?" this does not cure the problem. The car seems to need to be turned off for a longer time. If I turn the car off and return in, say, 2 or more hours, the issue is cured (temporarily). Attached are still images from a video. This is random in occurrence and seems to be completely unpredictable when it will will happen. Safety issue.
