This is the 2nd time we've had to replace the pulley for the serpentine belt and about the 6th belt. I have owned 3 Grand Voyagers and they ALL have had this problem for some reason; you'd think Plymouth would fix this issue after 10 years. This time the pulley came loose and got bent. The belt usually comes off when the pulley gets wet, like from rain or snow, when it's most inconvenient to be broken down on the side of the road trying to put it back on!!! I've researched this problem, and apparently there's a lot of people that have had this same problem.
