When driving consumer rearended a vehicle at 30-40 mph and neither driver's side nor passenger's side air bags deployed. Also, seat belts malfunctioned, causing driver to go into steering wheel. Frame was bent on front end.

Vehicle was rear ended by a dump truck, causing consumer to lose control. Vehicle turned around and hit a guardrail at about 50 mph. Upon impact, both passenger's side and driver's side airbags did not deploy. Consumer sustained injuries.
