Belt idler bearing must have failed, it wore a hole through the plastic cover and fell out of my car on the highway. Timing belt loosened up and caused bent/broken valves. Cheaper to replace engine at the shop than having the engine tore down and fixed. Timing belt & water pump were replaced at ~123,000 miles when the head gaskets were replaced.
