On 01-22-20, in austin Texas my daughter, [xxx], was killed in a head on collision while she was driving her 2007 Toyota Corolla. The other vehicle that was involved in the head on collision was driving a 1993 Chevrolet Silverado pickup. When the vehicles collided my daughter was killed because she was pinned against the steering wheel, unfortunately the airbags did not deploy and save her. The other driver received severe facial injuries. Why didn't her airbags deploy" why did her 2007 Toyota have a high safety rating but yet she was pinned be her steering wheel" 'parts of this document have been redacted to protect personally identifiable information pursuant to the Freedom Of Information Act (FOIA), 5 U.S.C. 552(B)(6).'

The contact owns a 2007 Toyota Corolla. The vehicle was purchased brand new in 2007. Two months after the vehicle was purchased, the factory alarm system engaged intermittently. The dealer repaired the alarm failure. In September of 2011, the check engine warning indicator illuminated. On February 21, 2012, a warning indicator illuminated. The contact stated that the vehicle rolled backwards without warning while parked. The dealer test drove the vehicle and determined that the vehicle was functioning as designed. The dealer produced a failure code detected for the torque converter clutch lock up and recommended that the transmission assembly be replaced. On February 24, 2012, an ECM recall repair was performed. Also in 2012, the contact noticed rust on the hood of the vehicle and small white circles (the size of basketballs) on the rear bumper and trunk. Two years later, leopard like spots were present on the roof of the vehicle. The manufacturer and dealer were notified of the defects. The manufacturer scheduled an appointment with the dealer in December of 2016 for diagnostic testing. The dealer determined that there were only paint chips on the vehicle and failed to include the extensive amount of rust on the roof. The service manager informed the contact that they would use oxidation on the vehicle regarding the rust. The manufacturer stated that the paint used on the vehicle was environmentally safe. In addition, the contact received a recall notice for the air bags. It was unknown if the notice pertained to the passenger or driver side air bag. The NHTSA campaign numbers and failure mileage were not provided. The VIN was invalid.

On March 12, 2015, at approximately 11:00 a.M, I received a phone call from sacramento police department stating that my vehicle was struck by a solid waste vehicle. I called the city of sacramento and even emailed the city of sacramento and I received no response. I picked up a copy of the police report on 04/01/2015. The name of the driver that struck my vehicle is [xxx]. the truck that struck my vehicle is a 2012 newway recycle truck (blue). According to the police report, [xxx] was distracted by a man who walked up to him and "started talking to him about a tv". I contacted [xxx] (integrated waste supervisor) and I was told that I would have to cover all expenses out of pocket including deductible, and rental car for the entire time it takes to fix my car. I explained to enrique that I did not believe it was fair that his driver struck my vehicle and I need to pay for it. Enrique said he was sorry but that was the way the city works. I told enrique that I may need to get a lawyer. Enrique's response was "go ahead". information redacted pursuant to the Freedom Of Information Act (FOIA), 5 U.S.C. 552(B)(6).

I was pulling into my parking spot at about 2 to 3 miles per hour as I was stepping on the brake pedal the car accelerated as if someone had push the gas pedal with 1000lbs of force. The car literally flew over the curve onto some bushes over the sidewalk sideswiped a light poll and onto the street the car slowed down when I was on the street. During this whole time my foot was on the brake pedal all the way down but the car would not stop, there are skid marks that show that and when I stopped my brakes where smoking and there was a very strong smell of burning brakes. Please advise me on what to do because I do not want to drive this car anymore, I am very scared.

As I approached traffic lights, I had to apply the brakes as yellow light was on but the car was still accelerating. Even after the brakes were completely applied, it went forward. To my bad, it was a snowy day in my place and my car skidded in to the left turn lane, got crashed in to the other car. My car's front bumper, head lights and couple of other parts were replaced for which I have spent $500 out of my pocket and the remaining amount was paid by my insurance company. Luckily there were no injuries to me, my 7 month pregnant wife and the other car driver. Obviously my insurance premium got increased, my driving history was affected and car value is decreased due to accident history. My car tires and brakes were in perfect condition when this incident happened. Now I am afraid to drive my car especially when it approaches traffic lights I feel extremely nervous. Whenever I recollect this bad moment I feel depressed. Please look into the issue. Requesting Toyota to do the needful to remove such defects from their vehicles.

I stopped at a gas station to fill my gas tank and while this was being done I heard a loud bang similar to a gunshot. When I started my car there was a loud rattling sound. I stopped the car and got out to look underneath but couldn't see anything hanging. Since I was near home I started the car and drove slowly home. I took it to the dealer the next day and they repaired the gas tank strap passenger side. I told them I considered this to be a design defection and a safety issue. They stated there has been no recall despite the fact that they had seen quite a few of the same problems. I had to pay for the repair. Toyota Corp has refused to reimburse me for the repair because my car is out of warranty. I have had many cars and have never had this problem before. I am told that there is a drivers side strap as well so I suppose I can look forward to this happening again plus since it is due to corrosion I can expect this to happen again in about five years. I do not know how likely this is to lead to the gas tank dropping but I don't feel to good about it.

In March, 2007, I had an accident. The rear end of the car was completely broken. I thought it was totaled. Safeco had it repaired. The trunk does not stay secure, I am continuously taking my vehicle to the body shop for repairs to the body. The body parts just pop in (plastic) and the trunk lid does not stay shut. I am very disappointed, as well as it being very dangerous when I am riding down the road and the trunk's lid flops open, causing blind visibility.
