I have reported this issue to the dealer. They tried unsuccessfully to fix this problem. They have reported it to Toyota. To the best of my knowledge, at this time Toyota does not have a fix for this issue. I like the car and it gets good mileage, BUT if I can not fill it up it will affect the miles I can drive between fill-ups.
I find this very annoying and I am very disappointed as this is the 5th Toyota we have purchased through the years and all the others have been great. We have a 2014 Camry Hybrid, we can fill it full and the gas gauge reads full. With this new RAV4 I never know how much gas is really in it!!
I feel Toyota either needs to fix this issue or refund my money. I am concerned about trade in value for this car as I believe this issue will become more common knowledge and I will not be able to get a good trade-in value for this car. I also purchased some expensive options for this car as I had planned to keep it long term.

The fuel tank will not fill past 3/4 full. The service dept at our local Toyota dealer noted the same issue. They contacted Toyota and replaced our gas tank. It filled to full the first time. And after that, we have the same problem.

No confidence in how much fuel is in tank. Being a hybrid, running out of gas can be catastrophic on hybrid battery. Issue needs immediate fix or vehicle needs to be replaced.

I am having the same problem as well. You have a legitimate issue. Although Toyota diagnostics shows everything is working, there is something wrong. Unfortunately, Toyota is still unable to root cause the issue.
You can validate if you truly have this issue. The Rav4 Hybrid has a 14.5 gallon tank. Multiply this by your average MPG. This number is where your Distance To Empty should be on a full tank. Also, a full tank should be above the F on the fuel gauge.

I am a Pacific Pride member so I have been pumping my own gas for many years in multiple vehicles including a rental 2018 rav 4 so I know how to fill a gas tank. After a 2500 miles road trip in this brand new rav hybrid I have not seen it show as full since I purchased the vehicle at 23 miles. It was full at time of purchase but hasn’t been since.
Update from May 23, 2019: Dealer states that they performed an "active test" and the fuel tank is performing to specifications.

It's hard to get the last few gallons into the tank when filling up. I'm getting decent milage but not "the distance" per tank full that I was told because I'm always "a couple gallons short" of the 14.5 it was shown as what I "should be able to" put in total.
