The contact owned a 2015 Toyota Sienna. The contact stated that the cruise control failed to deactivate when the brake pedal was depressed. As a result, the vehicle was involved in a front impact collision. The air bags deployed. The contact sustained a severe head injury, the front seat passenger sustained a broken nose and cracked ribs, and the rear seat passenger sustained minor injuries. Medical attention was required. A police report was filed. The vehicle was destroyed and towed to a salvage yard. The manufacturer was notified of the failure. The failure mileage was approximately 13,000. The VIN was not provided.
