The contact stated that while driving 15 mph she applied the brakes abruptly to avoid a crash but the seat belt failed to retract causing her face to hit the steering wheel, causing injuries to her lips, teeth, and chin. The contact owned a 2010 Toyota Yaris. The contact stated that the driver was driving 50 mph when another vehicle crashed into the driver's vehicle, causing the vehicle to crash into a cement wall. None of the air bags deployed and the front passenger seat belt failed during the crash. The crash caused head, neck, and chest injuries to the driver, front passenger, and rear passenger. The vehicle was destroyed. The police were contacted and a report was taken. The vehicle was taken to a local repair shop. The manufacturer was contacted.
