electric parking brake did not engage

The car is manual transmission which I have driven my whole life, and I'm no spring chicken. I live in a flat city where hills or rolling back has never been an issue. My car has an electric parking brake. One day about a year into the lease, I was parked at a pump at the gas station and when I came out, my car had moved/rolled back. Shockingly I thought I messed up and might've forgotten to engage the parking brake, despite it becoming 2nd nature. I thanked God it didn't roll back into anything, or anyone, and vowed to be extra, extra mindful and careful.

Turns out, no matter how careful and mindful I've been, just the other day, it happened again. This time, my husband was in the car with me and swore he saw me as usual, as 2nd nature, engage the electric parking brake. Mind you, I have never put it into 1st year to park because I've never needed to. I guess I should have this time as a back-up to the electric parking brake that didn't engage. We were across the parking lot at the ATM when we saw the car rolling back... my husband ran to try to stop it before it could hit something but he didn't succeed. It was too heavy and ended up crashing into the posts and handrail that I guess are there for that exact reason. The posts stopped my car but my husband hurt his wrist and neck in his attempt to be SuperMan. The posts of course had no damage.

Contacted Mazda, and they said they'd check it out. I referenced that there was a recall in prior years. They said no recall has been announced for this year. During this whole COVID-19 and my husband being furloughed, the last thing we need is an extra expense as my insurance deductible will be. The car is at the dealer now and they are testing it to see if it happens while in their shop. I hate for this to be a faulty part or warranty issue but also hoping it is so it will be announced and other people can learn about this horrible possibility. So grateful that the car did NOT roll back and injure a person or an animal! Ugh! Beware! Please share if you've had the same issue so we can get a re-call going. .
